This chapter is dedicated to my dear friend Mark Sadler (19452002), who passed away after a valiant struggle with ALS. Mark went to Oxford, and his college (Balliol) inspired him to be "effortlessly superior." He was, in so many ways. We often talked about the material in this chapter, and he encouraged me to propagate these ideas more widely.
