Title: Manual of Psychiatric Therapeutics, 3rd Edition

ACKNOWLEDGMENTS

For those who have never written or edited a book, the process may seem opaque . To the author or editor, the process can be rather perplexing; rarely does a project go smoothly ”snags are always encountered , and many deadlines are not met. Along the way, many people have facilitated the process of writing the Manual of Psychiatric Therapeutics , and I wish to single some out for special thanks.

Many people enabled both the writing and the production of this manual. I am particularly indebted to James V. Forgione, pharm.d., of the Drug Information Center at the New England Medical Center. Whenever I had reason to question whether a particular drug or dosage form was actually available, he cheerfully used his resources to track down such facts for me. Because of the revision of the chapter covering attention deficit hyperactivity disorder and the addition of a new and extensive chapter focusing on youth, a number of colleagues were asked to review early drafts and to provide feedback. Special thanks go to Drs. Pablo Davanzo, Bruce Fogas, Joseph Horrigan, Bennett Leventhal, Andres Martin, Gabriella Weiss, and Issac Wood.

My gratitude also goes to Charley Mitchell, my Aquisitions editor at Lippincott Williams & Wilkins, for his persistance and tolerance when I required accomodations because of my wife's illness and death. There is also a tedious aspect to the final stages of book production. Christiana Sahl, my production editor at Lippincott Williams & Wilkins, brought her skills and good sense of humor to this stage and not only kept me on track but also added some fun to this task.

I also want to thank my patients , trainees, and colleagues who have taught me over the years or who have challenged me to think about human suffering; the distress and pain we cause each other; the burdens imposed by stigma, prejudice, ignorance, and bias; the importance of forgiveness , tolerance, and owning one's own behavior; respect for individual differences; and the complexities of the human condition.
