Using and Adapting the Sample Evaluation Forms Provided in this Workbook

Each of the workshops contained in this book contains sample evaluation forms for Level One and Level Two evaluations based on the material as it appears here. You will want to modify these forms to reflect any additional learning objectives and learning content added (or removed) from your final version of the workshop. Use these forms as a starting point for evaluation. The sooner you can introduce Level Three and Level Four evaluation into your project management training program, the more you will be able to demonstrate lasting value and fine-tune workshops to reflect the unique performance requirements for project managers and team members within your organization.
