Ultrasound
A. Carotid and/or vertebral duplex
(AKA carotid noninvasives, CNIs): For stenoses in neck vessels. Uses B-mode and pulsed Doppler imaging.
B. Transcranial Doppler (TCD)
For stenoses in intracranial vessels. Specify anterior and/or posterior circulation TCDs when ordering. Add TCDs to CNIs if there is greater than 60% carotid stenosis on CNI. Can also help identify vasospasm, AVMs, absence of cerebral blood flow in brain death.
C. Neonatal brain ultrasound.
For bleeds: periventricular leukomalacia, and congenital malformations in pt. with an open fontanelle.
