A good tool for acquiring input regarding your business, product, and customer service is an after-sale survey. Wait a few days after customers have received your product or service, and then email a customer-satisfaction survey. The elements of the survey should include the following and should be just check boxes and/or radial buttons:
At the very end of the email survey, provide a comment box for additional comments. You can also take this opportunity to say thank you to the customer by including a discount code. This encourages repeat customers.
