All the training materials, PowerPoint presentations, and accompanying handouts, forms, training instruments, and evaluation forms appearing in this workbook are included in the CD that accompanies this book. Follow the instructions in the Appendix, ‚“Using the Compact Disc ‚½ and the document on the CD titled ‚“How to Use This CD ‚½ to access the materials on the disc.

The training materials in this book and CD include:
